Topic: Change of Address
Have you recently moved or changed your address since leaving Northwell?
Please submit a written request including the following information
Old address
New address
Last 4 numbers of your SSN
This information can be sent via email to retirement@northwell.edu, via fax to (516) 224-3180 or via mail to 1111 Marcus Avenue, Suite LL20, Lake Success, NY 11042. The purpose of updating your address is to ensure you receive your W-2, retirement information, and other important documents.
Topic: PTO Payout
For former non-union team members, once departing from Northwell you may be eligible for PTO payout. For former union team members please refer to your collective bargaining agreement (CBA) for the PTO policies and eligibility. Please note, to be eligible for PTO payout you need to have left in good standing, and provided sufficient notice. If you had direct deposit when leaving Northwell, payout will be deposited in the account we have on file. If you did not have direct deposit set up, you will receive a paper check on the mailing address we have on file when you left.
